IHCC (Nurse-Led Care Coordination)
Best Day provides nurse-led care coordination to help members understand their care, communicate with providers, address barriers, and follow through with needed services and resources.
Attendant Care
- Assistance with bathing, dressing, grooming and toileting
- Mobility and ambulation support
- Meal preparation and assistance with eating
- Support with routines and other authorized daily activities
Structured Family Caregiving
- Supports eligible members who receive daily care from an approved caregiver in a qualified home setting
- Best Day provides caregiver education, coaching, monitoring, and ongoing support
- Eligibility, caregiver requirements, service level, authorization and compensation are determined through the applicable waiver program and person-centered service plan
Homemaker
- Light housekeeping in commonly used areas
- Laundry and changing bed linens
- Meal preparation and kitchen cleanup
- Dusting, sweeping, mopping, and bathroom cleaning
- Support with other authorized household tasks
Ride Services
- Non -Medical transportation for authorized or privately arranged trips.
- Grocery shopping, errands, and approved community activities
- Assistance entering and exiting the vehicle when appropriate
- Advance scheduling and service availability are required
